Office Visits and Information
At your first visit you will be asked to complete and sign a history registration form. Please bring:
- An up-to-date medical insurance card
- Your present glasses
- Any present eye medications
- A list of all present medications with dosages
The office staff will be happy to assist you in every way with the process.
At each visit, your medical history will be updated, your vision checked and, with few exceptions, one or both pupils will be dilated with drops. Dilation of the pupils allows examination of the retina and other structures inside the eye. These drops may cause some temporary blurring of vision and sensitivity to bright light. In strongly encourage you to bring sunglasses to wear and have a friend or relative drive you home after each visit.
Office hours are generally Monday through Friday 9 AM to 5 PM. I am always available for after hour calls or emergencies through my office, (845)454-3030.
Patients are seen by appointment. New patient visits may require up to 2-hours; follow-up visits are usually one hour. We make every effort to stay on schedule, but emergencies may arise which require immediate attention. We regret any inconvenience this may cause you. I you are unable to keep an appointment we would appreciate at least 24-hours notice. This courtesy will allow us to schedule another patient at that time.
Fees and Insurance
Charges for office visits and surgery are based on the complexity of the service and the time required for your eye care. You will be asked to pay any co-payments or deductible not covered by your insurance. I accept Medicare assignment, as well as participate in many major healthcare insurance programs. As a courtesy, we submit your claim for your to both your primary and secondary insurance carriers. Therefore, it may take some time before your receive a final statement of any outstanding balance.
Location and Parking
Our office is conveniently located on the corner of Jefferson and Pine Streets: diagonally across from the intersection of Stizel Baseball Field and just two blocks north of Vassar Brothers Hospital. Patient and visitor parking are free.
